GP practices in Bolton are expected to join a primary care network (PCN) by July 2019.
Each PCN will bring together a group of neighbouring GP practices to cover a population of between 30,000 and 50,000 patients. The networks will be expected to provide support for each member practice, benefit from economies of scale through collaboration to provide services through their expanded primary care team, such as sharing staff.
We have created a series of documents to support our member GP practices with decision making about how their primary care networks will operate.
