Enhanced Access Service

Bolton GP Federation provides evening and weekend appointments on behalf of 7 PCNs across Bolton.

PCN Support

We currently support 7 PCNs in Bolton.

The networks provide the structure and funding for services to be developed locally, in response to the needs of the patients they serve.

Support to GP Practices

Supporting practices is a Bolton GP Federation priority.

Our role in supporting practices is developing and growing in response to what they need.

Experienced Nurse Network

Was a new pilot service developed in July 2020 in response to requests from GP Practice to support them with their nursing needs.

COVID-19 Vaccinations

In December 2019 we began the delivery of the Covid vaccination programme on behalf of five of Bolton’s Primary Care Networks.

Training Hub

We were approached by Greater Manchester Health and Social Care Partnership to run the hub following the involvement of our Lead Nurse and the need to upskill workforce quickly in response to Covid vaccination programme.

GP Streaming Pilot

We ran a 6-week pilot during May & June 2020 to help reduce demand on A&E.

Intermediate Care

Has been running since 2017, providing clinical leadership and GP support to deliver enhanced care in care homes.

Mobile Health Hive

Our Mobile Health Hive is based on the Bolton Health Hive in Market Place, Bolton, offering support and services you would normally access through your GP practice.

Enhanced Access Service

Bolton GP Federation provides evening and weekend appointments on behalf of 7 PCNs across Bolton. Patients can book appointments in one of our hubs by contacting their own GP practice.

Saturday – 09.00–17.00
Monday to Friday – 18.30–21.30

Primary Care Networks support

Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan, with general practices being a part of a network, typically covering 30,000–50,000 patients. They are small enough to provide the personal care valued by both patients and GPs, but large enough to have impact and economies of scale through better collaboration between practices and others in the local health and social care system.

Support to GP Practices

Supporting practices is a Bolton GP Federation priority.

Our role in supporting practices is developing and growing in response to what they need.

We currently support them by:

  • Delivering Extended Primary Care – by providing evening and weekend appointments on behalf of practices.
  • Providing an Experienced Nurse Network for practices needing nurse cover.
  • Facilitating Practice Manager peer support.
  • Providing Practice Manager capacity support/cover.
  • Supporting care homes through multi-disciplinary team meetings.
  • Promoting practice job vacancies.
  • Provide Ardens Manager, an evidence-based resource that promotes best practice, medicines management and patient safety.

Following consultation with and requests from GP Practices, the Federations Experienced Nurse Network provides:

We provide a range of cover to practices, including:

  • General Practice Nurse cover
  • Cervical smears
  • Child immunisations
  • Flu vaccinations

Practices have found this service most helpful to support them with:

  • Filling recruitment gaps
  • Covering whilst their practice nurse is on holiday, sick leave, or in training, etc.

COVID-19 vaccinations

We deliver the COVID vaccination programme on behalf of five of Bolton’s Primary Care Networks; Central, Chorley Roads, Farnworth & Kearsley, Halliwell and Rumworth.

Training Hub

Our vaccine training hub is one of the first hubs in Greater Manchester and the only one in Bolton. It is led by Primary Care Workforce Development Team at Greater Manchester Health & Social Care Partnership.

Since 2017 we have provided clinical leadership and GP support to deliver enhanced care in care homes.

We provide this support to 12 beds in Wilfred Geere House and lead the multi-disciplinary team which is a dedicated team consisting of care supervisers, social workers, pharmacists, OTs and Age UK Bolton.

GP cover is provided Monday to Friday.
