We are looking for a registered Enhanced Practice Nurse to join our Primary Care Network team. You must have experience in working within general practice and primary care network. The duties include teaching and advising patients on how to manage their condition, delivering enhanced clinical care in the context of continual change, challenging environments, different models of care delivery and innovation.
