In order to help our GP practices work more closely together and to efficiently deliver projects such as extended access to primary care, we have clustered them together into groups that we call neighbourhoods. This approach is in line with what Bolton Council and Greater Manchester see as important to helping communities benefit from the devolved control that the region has been given.
Our neighbourhood teams are developing exciting plans for future services that are based on the needs of their own, distinct patient populations, so that patients will find services are properly tailored for them – this is a “patient-centred approach”. Your GP is part of a team that includes representatives from community and voluntary organisations, the Council and others who are all focused on what the people in each neighbourhood want and need to optimise their health and wellbeing.
