Primary care is facing many challenges today and demands on practices are greater than ever before. Our ageing population needs complex care and our practices need to offer a range of services in order to keep people out of hospital, at home and enjoying life.
Our work is focused on helping our members to find new ways of working that bring all the skills of health professionals into general practice, helping patients get the right care from the right person at the right time, while using scarce resources such as money and time more efficiently.
We are working with our member practices to develop an innovative place-based approach to meeting demand for greater access and more services. Our “neighbourhoods” programme is bringing practices together in nine areas where we believe that sharing some central services will bring benefits to both patients and the practices themselves.

On the 1 April 2016, the Greater Manchester Health and Social Care Partnership took charge of the £6bn health and social care budget from central government. The shared vision across Greater Manchester is to see the greatest and fastest improvement to the health and wellbeing of the 2.8 million people who live in Greater Manchester. ‘Taking Charge’ is a 5 year strategic plan for Greater Manchester built up from individual locality plans developed by the 10 local authorities and NHS organisations across the city region.
